Derby Diva/Queen Contest
Minor’s Release

We do herby consent and agree that it is satisfactory for our minor daughter,
____________________________, to compete in the 2008 White Dove of Hope Derby Diva/Queen Contest, and we do not hold White Dove of Hope, Inc responsible for any liability for bodily injury or any other damage or loss sustained or suffered while a participant in any of the events listed hereon.

It is also agreed that by signing this release, the contestant gives permission to use any pictures for video presentation, catalog, newspaper or web page without compensation or prior permission of contestant.

It is also agreed that by signing this release, we acknowledge we are assuming financial responsibility for any Demolition Derby Raffle Tickets that she has checked out to sell and we understand that either the tickets or the money must be turned into the White Dove of Hope no later than Thursday, April 24 by 7:00pm.
